Dubrow joined The Real Housewives of Orange County in its seventh season and appeared for five seasons (up until its eleventh) but stepped back from her role as a 'Housewife' to focus on spending time with her kids and family. Yet, after a decline in the show’s ratings, Dubrow was offered to re-join the show for its sixteenth season and has remained on the series since; having starred on the show’s seventeenth season and is starring in the show’s eighteenth too.

Heather is back living the dream in her 22,000 square foot home in Crystal Cove with her Husband, Terry, and four children. Their kids are starting to look at colleges and find their own voices, especially Max, who recently wrote a book for parents about teenage sexuality. After many years, Heather and Shannon reunite and instantly find themselves at odds over loyalty and betrayal. Heather also introduces a friend of hers to the group only to find out a shocking truth behind who she really is.

Heather is introduced with her "picture perfect life" married to plastic surgeon, Terry Dubrow, and a stay-at-home mother to her four kids. She decides to break back into the acting world after taking a step back to concentrate on expanding her family but feeling that the time is right to get back to her true passion.

The marriage between the Dubrows is under slight strain as Heather feels that Terry doesn’t prioritize her career and places his work as superior to hers. There are a couple of clashes between the pair which leads to the word “divorce” being thrown out.

While continuing to expand her career as an actress (after landing a guest role on Hawaii Five-0), Heather begins to plan and oversee the construction of her new family home.

Heather releases a sparkling wine, named Colette, after her youngest daughter (at the time). In addition to this, Heather continues to oversee the building works of her house and feels a disconnect once again between herself and Terry.

The construction of Heather and Terry’s new house is still underway while Heather still feels that Terry is mismanaging his priorities. As the Dubrow family go on holiday to Turks and Caicos, the kids notice the distance between the couple. Heather also documents the release of her new book.

As Heather re-joins the fold, she chooses to document the release of her latest book which helps celebrate the LGBTQIA+ community; which three of her children are involved with. Heather’s eldest children are also soon off to college so hunting heads underway.

Heather has a charity event with Family Equality, they are hosting a fashion show where the ladies will participate in in Sonoma. The vision of Family Equality aligns with Heather's as Heather's children range across the sexuality spectrum. Heather partners with Equality Wine and Family Equality, creating 4 sparkling wines each based on Heather's kids.
Heather is invited to the 35th GLAAD Media Award to present Outstanding Reality Television Competition Program Award along with Meredith Marks in Singled Out. Alexis is also in attendance, it is a cause close and dear to both of them. Heather won a bottle of champagne for $6,500 in the auction.

Heather is uneasy and taken aback that the other ladies did not inquire how her mammogram went.

Heather believes Tamra and puts a pin in it due to the fashion show starting.
Shannon facetimes her daughter Adeline in The Elephant in the Room, who's studying at NYU. Shannon told Adeline her tour with Vicki went well, and it was a well needed break from John's lawsuit. Shannon feels lonely as an empty-nester and is now forced to face the ongoing lawsuit again. People magazine reports the lawsuit publicly, Tamra sees the article and calls Shannon, who did not pick up. Shannon butt dials Tamra, rather than hanging up Tamra listens to Shannon's break down to her team. Tamra overhears Shannon thinking Alexis is behind the article release. Tamra then calls and updates Heather, Tamra doesn't think Alexis is behind the article. Heather states "John has historically said he doesn't like to be in the spotlight. This (People magazine article) would be counterintuitive to that". Heather doesn't understand Alexis and John's rationale and feels bad for Shannon.
Shannon passes out presents as a thank you to the ladies for flying out to London to celebrate her birthday in High Tea & High Tension. The presents include a $595 Burberry scarf, Tamra and Heather are appalled and thinks Shannon should use the money to pay John Janssen instead in their confessionals, but thank and hug Shannon in person. After Shannon leaves, Tamra brings up Shannon's appearance on Jeff Lewis' radio show. Where Shannon appears to admit borrowing money from John Janssen. Tamra and Heather are upset with Shannon as they think Shannon is telling a different version of the story. Heather tells Tamra about Shannon confiding with her in Sonoma and showing her post car accident photos. Heather is upset with Shannon responded with she "hasn't really thought about it" on Jeff Lewis when asked why she thinks John did not come check on her after hearing the car crash. In Heather's perspective contradicting with what Shannon had told her in Sonoma. Heather feels "used" and Tamra thinks Shannon is "using emotional manipulation" on Heather to play victim. At dinner, Tamra inquires the ladies about the mammograms, Heather delivers the bad news of her near 40% chance of getting breast cancer, and states she feels hurt the ladies did not check in with her post procedure. Tamra explains to Heather that a guy at her gym informed her Ryan was named in a lawsuit for timeshare fraud abroad, accusing Shannon of telling her she to look into it. Leslie Beador (Shannon's ex-husband's new wife)'s ex-husband is one of the defendants. Tamra is adamant Shannon is orchestrating behind the scenes and states she will not tell Jenn the truth because Jenn was yelling at her. Heather compliments Jenn's stomach and Tamra says Jenn had a tummy tuck and has "no belly button".
Heather and Tamra refresh Gina on Jenn’s visit over caviar. Gina expresses worries to Tamra, afraid that if she’s doing this to her so called friend, Gina might be at the receiving end of such behavior one day. Tamra told Heather that Kaili, who babysat Emily’s sons Keller and Luke, told her the boys said Heather is “mean” and “bossy” during candle making class. Gina does not believe in Kaili and thinks Katie made up the lie, stating Emily would not talk about the ladies around her children. Tamra continues with when she met up with Emily and Gina for breakfast, Emily contradicted Kaili’s story by saying her boys said Kaili was the one who said Heather is “mean”. The accusation came from a Page Six article, Taylor’s husband John refused to get involved. Heather asks Tamra if she’s going to tell Gina, they are going to sleep on it. At their own suite, Shannon appreciates Gina and Emily backing her up at dinner. Shannon resonates with Gina regarding her concern earlier at caviar with Tamra and Heather, wondering if Tamra is going to dredge up Travis’ past and throw it in Gina’s face.

The next day at dinner at the countryside, Heather asks if Shannon is upset about something and the two breaks away from the group to talk privately on the patio. Shannon confronts Heather about why she told the ladies her confiding with Heather in Sonoma in detail at the bar two days prior when she explicitly told Heather to not bring it up to the group. Heather responds to Shannon’s clear question with “should I respond now?” and proceeds to call Shannon’s accusation “bullsh*t”. Heather states she does not remember Shannon telling her to keep it in confidence, flashback disapproves Heather’s theory. Shannon realizes she should have confided with Gina instead. Heather is infuriated when Shannon states she did not feel “compassion” from her. Heather brings up Shannon and David’s bitter divorce and their fight in 2014 that culminated to her kicking Shannon out of her house. Heather thinks Shannon controls information and when she’d like people to get a hold of it. They argue and Heather ends up apologizing.
Shannon is "not happy" with Heather's change of opinion of her car accident picture in London in Reunion Part 1. Heather is defensive and states she only did after the Jeff Lewis radio show. Shannon says she was tired during the interview and tries to spin her statement unsuccessfully. Heather doesn't understand Shannon still confided with her privately. Heather visits Shannon's trailer during lunch and tries to clear things up in Reunion Part 2. Heather, Tamra, Shannon, and Gina gather at Emily's in Reunion Part 3. Heather reveals that five months earlier, Alexis, Heather, Tamra, and their respective spouses all went to dinner after the finale party. Heather tries to explain that Shannon wasn't the topic of conversation, but it came up, Gina doesn't believe her. John said that he did hear the crash on the night of Shannon's DUI. Gina and Emily reassure Heather that she had done the right thing telling Shannon. Tamra reveals John wanted to see what happened but his daughter told him to stay put and went herself instead. She discovered Shannon "pulling off". Additionally John said Shannon "hit him" that night. Shannon knows John said that she "punched him" but does not have recollection of the night. Shannon cries and is scared of her actions. Gina consoles Shannon, telling her even if she did "terrible" things that night, it doesn't make her a terrible person. Everyone join in and comfort her. Emily questions Tamra about how long that has been sitting on the information. Heather steps in and explains it was a lot of things happening. Everyone comfort Shannon, Shannon is grateful.

In Dinner Party Disaster, Heather is upset that Gina, being her friend would let Katie gather information and confront her. Heather interrogates Gina on her friendship with Katie, Gina apologizes to Heather. Heather feels betrayed by Gina and is angry at her from the Katie situation in All Up In Gina’s Grill. Gina feels Heather has been distant from her and is holding onto the Katie situation. At dinner, Heather and Gina argue. Heather proceeds to compares her paparazzi rumor to Shannon's comment about CPS and Gina's children last year. Specifically regarding how it affects her kids. Argument goes back to Katie, where the timeline of when Gina calls Katie after the Golf Event is disputed. Heather and Gina get emotional and agree to move on.
Katie tells Jenn that she wants to apologize again to Heather after being shut down by her at Tamra's Traitors Party in Catwalks and Catfights. Katie thinks Gina has been stirring the pot between her and Heather. At Heather's fashion show, Heather tells Tamra about Katie attempting to talk to her. Tamra plants the seed of Gina is manipulating Katie in Heather's head. Katie told Tamra that Gina told her she wants to stay on Heather's good side for her real estate connections, therefore letting Katie confront Heather regarding the paparazzi situation. Jenn tells Gina Katie tried to talk to Heather at the sound bath and insinuated Gina is the instigator. Gina tries to talk to Katie and Katie doesn't want to talk right away. The next day, Katie tells her husband Matt she's not looking forward to the talk with Heather and is upset with Gina.
The next day, Gina addresses the elephant in the room as they sit down for lunch. Katie states she has heard from people (Tamra) that Gina is calling her a liar. Gina explodes on Katie, and subsequently Heather. Emily defends Gina due to the rough patch she's going through. Heather brings up that Katie said Gina is only using Heather for her real estate connections, Gina is flabbergasted and denies. Emily affirms what Gina said. Katie and Gina argue. Katie reveals what Gina said was that Heather is a good friend that helps with her career. Emily tries to defuse the situation, Tamra tells Gina she's being defensive, Heather believes in Gina and Katie has not taken accountability.
Heather tells Terry about wanting to speak to Gina about the Katie situation in The Elephant in the Room. Heather, Shannon, and Katie go shopping for an outdoor patio sofa for Shannon in You Are Cordially Not Invited. Heather tells Shannon and Katie about her dilemma with Gina leaving with Emily at Jenn's Fiesta Party and asks for Katie's opinion, Katie see both sides. Emily visits Gina and tours the kids' remodeled bedrooms in You Are Cordially Not Invited. Gina feels uneasy with Heather. Heather is apparently mad at Gina for leaving with Emily after their conflict at Jenn's Fiesta Party.

The ladies are incredulous and worried for Jenn. After the mammograms were taken, Dr. Lopez announces Jenn's lump is not cancerous. Heather's results were clean, but is warned due to her dense breasts, her estimated risk is high at 39%, compared to 12% of her age group. Heather is uneasy. Gina and Shannon both receive clean bills of health, the ladies celebrate in the waiting room, although Heather is taken aback that the other ladies did not inquire how her mammogram went. At dinner, Tamra inquires the ladies about the mammograms, Heather delivers the bad news of her near 40% chance of getting breast cancer, and states she feels hurt the ladies did not check in with her post procedure. Shannon, Gina, and Jenn apologize, but Gina doesn't understand why Heather waited to bring it up instead of just calling her after the mammogram. Gina also points out the hypocrisy of Heather calling her out regarding the Katie situation.
Gina and Jenn visit a spa in Double Decker Drama, Gina feels home in London as it has a similar vibe to New York. Gina tells Jenn about Shannon showing her the texts Alexis sent her in 2018 that contradicts her saying she was not involved in Jim Bellino's defamation suit toward Shannon and Tamra. And Shannon had informed Tamra about the texts, Gina is incredulous that Tamra continued to attack Shannon at dinner while possessing such information. Jenn thinks Tamra is befriending Alexis solely for the purpose of taking down Shannon. Gina feels Tamra and Heather are hyper-examining every single move made by Shannon. They don't understand how Heather turned Shannon confiding with her into about her, they are also confused by Heather's upset over mammograms, as she informed the ladies over text to not mention it on camera due to her family history. Gina tells Jenn her Dolce and Gabbana at Shannon's 60th Birthday Tea Party became a topic of contention, as Heather saw the same dress at Neiman Marcus and called Tamra to inform her the price tag of $2,000. Flashback shows Heather, Tamra, and Emily discussing it on the Sprinter Van and stating they would not be spending $2,000 on a dress while in financial trouble. Tamra explains to Heather that a guy at her gym informed her Ryan was named in a lawsuit for timeshare fraud abroad, accusing Shannon of telling her she to look into it. Leslie Beador (Shannon's ex-husband's new wife)'s ex-husband is one of the defendants. Tamra is adamant Shannon is orchestrating behind the scenes and states she will not tell Jenn the truth because Jenn was yelling at her. Heather compliments Jenn's stomach and Tamra says Jenn had a tummy tuck and has "no belly button". Later on at their suite, Heather encourages Tamra to properly apologize to Jenn at the countryside tomorrow in order to move forward.

In Not My Cup of Tee, Katie revealed that her friend who has close connections with paparazzi confirmed Tamra and Emily's suspicions, last season, of Heather hiring paparazzi at Disneyland. Katie brings up Heather tagging her in a picture that she is not in, which she had confronted Heather with at a nail spa. But Heather explained that her social media manager must have accidentally tagged her as she was wearing a Givenchy dress and Katie's last name is Ginella. Katie and Heather talk later at Katie's golf event, and Heather swears "on the lives of my (her) children", she had never called paparazzi when Katie said Heather lied. The ladies joined the conversation. Heather refuted gathering information on Katie and said that Katie is the one that is gathering information on her.
Katie states she had sent Heather a text over a week ago but hasn't heard back while doing yoga with Jenn in Once A Traitor, Always A Traitor. Later at Tamra's Traitor party, Heather pulls Katie aside to extend an invitation to her upcoming Sonoma trip. Katie was named in a $5 million lawsuit over stolen ideas of her business Tee Shots. Katie states she has been dismissed from the case, she was sued by her business partner and her husband's friend. Allegedly he had a conversation with her business partner's husband on a golf course about the idea. Heather jumps in and brings up a text from Sutton that states Katie used her for an event, didn't fulfill her promise of bringing in high level golfers and didn't pay for the photographers. Katie refutes with she did offer to pay for the photographers, and the caterers as well. Heather snubbing Katie at Sutton's Christmas Party is brought up. Heather is excited to expose Katie's three variations of the stories. The first being her snubbing Katie at her drive way. Katie defends herself and Heather shuts her down. The second story is that it happened at the bar, which Katie interjects as the real version with Denise Richards and Camille Grammer also present. Heather states that is a lie as she never saw Camille that night. The last story being Heather was with a group of people and with Katie's name brought up and she did not have a reaction. Heather is furious at Katie, telling her to "let it go" when the topic has not been discussed in months. Heather thinks Katie has an agenda against her and "has absolutely no credibility". Katie apologizes to Gina for thinking that she set her up, also bringing up that Tamra planted the idea in her head. Katie states she wanted to talk to Heather one on one at the Golf Event instead of making it a group thing. Gina reminds her that she talked about it with the group at the bar. Heather goes off on Katie and Katie states she didn't think it'd become a huge thing. Heather states "falsely accusing people is kinda your thing". Alluding to the time where Katie falsely accused someone and they got arrested because of it.
Katie wants to clarify, Heather and Emily don't want to hear it because Katie is inconsistent. Andy lets Katie have the floor. About 10 years ago, Katie was driving her 3 little kids on a 4 lane road in Marietta, Georgia. A man swerved into her lane and shouted "f*ck you b*tch". Katie pulled over to a side road, where the man followed and goes to the front of her and screamed at her. Katie called the police in fear of her and her kids. Katie tried to back up and leave, the man went to the back of her car to prevent her from doing so and started filming. Heather interrupts with that's not what the police statement indicates. Katie continues with the police arriving and attempting to calm the man down. They tase him after warnings and he was taken to jail. Heather states the police said Katie followed the man to his home, per police report from an in Touch article. Katie retorts with the police statement is the man's point of view and she has her own police statement.
Heather continues with what she read from the article: Katie was texting and driving with her kids in the car. The man saw, he was triggered as his family member passed away from a texting driving incident, therefore he told Katie to stop. Katie road raged and followed him home. Katie doesn't understand the rationale of her following a stranger to their house with her kids in the car. He started filming her as she got out of her and yelled at him. Katie called the police, and when they arrived she told them the man won't let her leave. The police took Katie's side, threw the man to the ground, in the process injuring him, and took him to jail. He was released when the police realized Katie was the "aggressor'.
Katie states she could not tell her version of the story because she did not show up in court. She was automatically guilty due to her absence. The court order was served at Katie's ex-husband's as that's her address on file. At the time Katie was homeless with her kids. Gina expressed incredulity and the court order has to go to her and not just to an address on file. Emily thinks there's an excuse every time Katie's accused. Shannon is oblivious to the incident while everyone else knows. Andy points out the irony of Heather stating earlier that she doesn't like the "digging stuff on people". Emily defends Heather because the article is on the "public domain".

Emily brings up an article about Katie's divorce, where Katie lost both physical and legal custody of her kids. Katie explains that she was living at a "facility" 20 minutes away from her parents, who didn't want to help her as she didn't agree with her divorce. She lost custody because she was not able to provide for her kids. Emily exercises her lawyer knowledge and questions that legal custody should not be lost. Katie doesn't have an answer for Emily, but thinks at the time she was okay with relinquishing it since she couldn't provide for her kids. Katie had to rebuild her life in order to go to back to court multiple times to restore custody. The kids were in her ex-husband's custody for 18 months. Katie is emotional and states she had never missed an appointment with her kids through the 18 months. Katie cries and states "I hate talking about my ex-husband". Heather expresses sympathy to Katie directly during break.
It's revealed in Reunion Part 2 that on the flight to London the turbulence was so bad Heather and Katie were holding hands. Jenn also clarifies Ryan bought her her tea party dress. Heather defends herself by bringing up that Jenn had called her prior to the season starting asking for financial advice, but did not follow any of it. Katie calls Heather out for her behavior or checking the price of Jenn's dress at Neiman Marcus as "mean girl behavior". Heather thinks it's just "gossiping" with the support of Tamra, Emily, and Gina.
Katie apologizes to Emily, who does not accept her apology. Gina, Emily, and Heather mention Katie saying she only talks about Housewives stuff with Kaili since she's an adult. But footage shows Bandon asking Katie if Heather is mean. Katie explains that Bandon overheard her coming home crying after her Golf Event. Andy is incredulous as everyone talk in front of their kids. Gina, Emily, and Heather refute. Katie apologizes multiple times, Emily is still mad that she brought her kids into it, but seems more mad that her friendship with Heather is potentially jeopardized. Heather is furious that Kaili is making TikToks shading her. Specifically "fake a*s b*tches who suck dick", however, Kaili did not say that, she was doing a lip sync. The ladies are disgusted, even though Kaili is 20 years old and Katie is not responsible for her actions. Katie states she did ask Kaili to take the video down. The ladies are incredulous on why Kaili posted the video, seemingly amnesiac of them coming for Katie and calling Kaili a liar. Andy moves on and doesn't want to keep beating a dead horse.

Heather accompanies Terry for his pre-operation tests for his nose job in The Elephant in the Room. Terry is getting his deviated septum fixed, with Heather complaining of his snoring. Heather tells Terry about Jenn's Fiesta Party, as well as wanting to speak to Gina about the Katie situation. Terry tells Heather she processes conflicts multiple times in order to get through it, to the degree of excess. Terry does not want to continue the conversation, leading to Heather's hot mic telling Terry "don't admonish me on camera". Regardless Heather helps Terry with his hospital gown as the doctors move on with the tests. Heather is still nervous due to Terry's mini-stroke last year. On operation day, Heather is worried while Terry comforts her with jokes. The surgery went well.
